MT Community Needs Assessment

[MT-PRO Music Therapy List]

Has anyone used or developed a survey for determining the interest, 
knowledge, needs of a community?  I am with a community music school 
and am 
interested in doing a survey of this area.  Also, a colleague is working on a 
grant to fund a needs assessment for community MT services.
